Please complete the survey for each student interested in participating in the 2022 Kurwongbah SS Choral Program. Enrolments are open to all students in Year 2-6.

Completed forms (SRS Participation Agreement Form and SRS Fee Payment Arrangement Form) can be returned once school has resumed. If you are unable to print the forms you can collect them from the office or music room, once school resumes.
